Administration of Prescription Medication

Our policy and procedure has been revised to align with the Ministry of Education’s NEW PPM 161: Supporting Children and Students with Prevalent Medical Conditions (Anaphylaxis, Asthma, Diabetes, and/or Epilepsy). As such, we have revised our forms. Attached are the NEW Authorization for Administration of Daily Prescription Medication forms.  

Please note: there are two forms that each have a different purpose.

1)    Authorization for Administration of Daily Prescription Medication Form: This form is needed if your child requires prescription medication, at school, as prescribed by a medical practitioner. There will be a requirement for an Individual Plan of Care to be completed in addition to this form.

2)    Authorization for Administration of PRN Prescription Medication Form: This form is needed if your child requires prescription medication as a PRN, at school, as prescribed by a medical practitioner. A PRN (meaning “pro re nata”) is a prescription medication that is administered to a student on an “as needed” (PRN) basis, as specifically outlined by a medical practitioner; these are rare circumstances and are for specific, identifiable symptoms that require the medication. There will be a requirement for an Individual Plan of Care to be completed in addition to this form.

 

The Individual Plan of Care forms will be available in hard copy from the school in September and electronically in the Parent Portal.
